Bitcoin Diamond successfully forked at block height 495866 on November 24th 2017. Which wallets and exchanges support the HF is unclear at this moment. However if you held Bitcoins at the time of the snapshot, you should be able to get BCD in the near future. You get 10 BCD for each Bitcoin you held during the snapshot.

Both Ledger and Trezor wallets stated it is not in immediate plans to support this hardfork.
